Chronic Care Program
Our Chronic Care Program allows TC2U to serve as a trusted primary care partner for your patients, offering consistent, reliable support directly in the home.
Care begins with an initial telemedicine visit with one of our physicians, assisted by our field staff to ensure a smooth start. From there, our nurse practitioners provide bi-monthly in-home visits, helping manage chronic conditions and easing the burden of outside appointments for both patients and their families.
To further support continuity, our nursing team completes follow-up phone calls every other month. These touchpoints help us stay ahead of emerging needs, reinforce care plans, and make sure nothing falls through the cracks.
Transitional Care Program
Our Transitional Care Program supports patients in the critical first 30 days after discharge from an acute-care facility.
Every patient receives an initial telemedicine visit with our Medical Doctor followed by multiple touch points throughout the month, including in-home visits from our Nurse Practitioners and scheduled follow-up calls from our nursing team.
